Need help what can happen to clutch disk be worn on splines

Like what can cause it or what could happen if they fail? If they fail the car doesn't move and hopefully it doesn't destroy the input shaft in the transmission.

It looks like someone tried to force the transmission onto the engine without everything lined up properly. Definitely replace that clutch. How does the input shaft look? The pilot bearing?

Very strange done numerous clutches in my life but newer seen anything like that. Perhaps incorrect clutch plate has been installed..?

If you are talking about the recess before the splines, that is normal.
