Use ISTA/P and ICOM interface. Select Replace when ask ISTA/P and everything do it automatically.

Ive ISTA/P installed, dont have ICOM only pass thru but can not get ICOM working with passhru

Yeah, offline with loader don't work with PassThru, unfortunately.
But you've mentioned you've got K+D Can cable. ISTA/P can work with that too when using Loader v6.1.
You can give it a try.

With NCS Expert it is easy job, use K+DCAN + NCS. 2 minutes job.

Where do I find the BMW standard tools, I have downloaded, I have folders:

Bmw tools
EC-APPS
EDDIBAS
NCSEXPER

but 2 files missing, the install instructions and the St212 files?

Look for Sedoy's setup file in the pinned post here - ISTA Standalone / Standard Tools / PSdZData_Lite - last [MEGA]

Start installed apps from Start menu shortcuts.
