Can you please advise. Do I have to do a new install on fresh OS? OR, I'll be fine with just uninstalling the old one, and installing the latest?

If you’re running Windows 10 64bit you could do either way.
Request the license after installed new ISTA.

I just offered to make available a mega link to download the latest version or previous versions of BMW ISTA for those who don't have the link or are having trouble finding it   As for installation you have to use windows 10 for latest version of BMW ISTA just that. The license follows the guidance of the friend there. 
As for the installation if you are using windows 10 just uninstall the old version completely and install the latest version. I apologize for the misunderstanding ok

I also have problems connecting to 2 different G01 chassis vehicles. I have an ICOM and it will read the VIN and PAD status on the menu bar. But after vehicle ID, it starts asking me to change the PAD status and it will show the status on the menu bar, but ISTA will still ask me if I have changed the status. In the end, all modules show red and no communications with the modules. I have tried uninstalling and installing an older version but same thing. My ICOM works on another laptop fine, so I think it must be my laptop or the install. Any ideas how to fix this? Thanks.

Would be much easier if you would provide ISTA log. Or do you want us to play guessing game?

Well, then my first guess: don't know if it troubles G series, but for F series you had to set "ethernet as preferred communication channel when connected via ICOM" to no in ISTA. Especially if your ICOM is a clone and has faulty/missing car ethernet.
