Finally resolved. I believe the d-flash was too corrupted to repair, maybe by a previous repair elsewhere or faulty CAS.
The solution was to right a new d-flash and adjust vin, mileage FA etc.
What made things worse though, the new key didn't work! Programmed a 2nd new key, all good.
