for the smoke, switch of your van. take the egr plug off. and then check.
if it still smokes. your egr is stuck open.
you will need to get blanking plate and fit it before or after egr. (you are blocking egr gas to go into engine)

if that doesn't help, check your turbo. sometimes something gets stuck in the turbo fan or there is a pipe leak somewhere from the turbo to the cooler to the engine. rev the vehicle and put your hand on the pipes. you will feel a lot of air on your hand. and some times when you rev you can hear whistle(pipe leak)
